Patient Privacy - We are required by applicable federal and state law to maintain the privacy of your health information.  We are also required to give you this Notice of Patient Privacy about our privacy practices, our legal duties, and your rights concerning your health information.  Please print and carefully review the Notice of Patient Privacy then acknowledge your receipt of this Notice by printing and signing the HIPAA Authorization Form attached.

Patient Registration, Health History, and Medication List – these forms, provided here as a single document, are very important to print, complete, and bring to your initial visit. 

Patient Records Release Request Form – if you would like records transferred from another dental office then please complete this form and send to your previous dental office to release records to our office.
